Primary Care 'Tsar' is a stupid fuck


Ok, first off, I really hate the way NuLabour has a 'tsar' for every problem. The only 'tsar' I want to see is a 'How The Fuck Do We Get Rid of These Evil Lying NuLabour Toadies Before They Totally Destroy Health Care in the UK' Tsar.

But, what really fucks me off is that the Tsars are so bloody stupid. I mean, take Primary Care Tsar Dr Colin-Thomé's plan to end patient reviews by consultants (as reported in the Guardian and by the BBC).

What a stupid fuck.

I mean, this guy is a doctor. So, he is either a really, really, crap doctor - or he is a lying shit.

Let's look at why:

1. Patients need to see consultants for review so the specialist can pick up subtle problems, answer complex questions, and so on.

2. Specialists need to see patients for follow up so they can learn from their successes and experience - this is basic to professional training and on going education. You know, the reflective, life-long stuff that means that patient get good safe care.

3. What does he think GPs do all day? Sit around drinking tea and twiddling their thumbs? GPs are just as expensive per hour, and just as busy, as their consultant colleagues.

4. Dr Colin-Thomé is a lying fuck if he thinks that resources will be diverted to GPs to let them take up this extra work. None of the other 'care in the community' has been funded - it's just dumping work into an already over-stretched part of the NHS because it's cheaper.

And his calculations for potential savings are something a half-starved retard would be ashamed of:

The Department of Health calculates the changes could save around £1.9b a year..

The calculation appears to be:
31.5 million consultant follow up appointments at £90 per time with consultant, but £30 with GP. Difference is £60. Multiply by 31.5 million and you get approx £1.9 billion.

This is hopelessly simplistic, not least because it assumes that GPs are cheaper than consultants*. Also, to make any sense, you would need to believe that being a surgeon is just about cutting people up. They are forgetting all of the other stuff - like working out when and how to cut, teaching the next generation of surgeons when and how to cut, and sorting out the problems that occur before and after the cutting. Top tip for NHS Tsars: the NHS is not a fucking butchers shop.

So, lets shit all over patient safety, standards of care, education, and continuing professional development. Yes, great idea. Clusterfuck the NHS to save a few million to pay for the £20+ billion you and your assbrained colleagues at the DoH have squandered on useless 'big' ideas.

As one doctor said on DNUK:

Why do we call them Czars? Because we hope one day someone will shoot the little fuckers.



Notes:
* This only true when you take into account differences in case complexity and support services, which should be the same if the service is transfered to GPs. But of course there won't be more time and more support services available to GPs to see the same patients (otherwise, where would the savings come from?) - ie: you save money by providing a cut down, crap service.

Ah, but if you read the news reports, the Czar is not suggesting that all cases be followed up by a GP. He is suggesting that cases be followed up by a GP or nurse, and nurses are cheaper. So there you have it: why train a surgeon for all those years when a nurse can do the follow up appointment?

And the impression I get is that there will be no automatic follow up appointment, but that it will be down to the patient to contact their GP or practice nurse if they have concerns. I hope I don't need surgery under the new regime.

As a 'mere' patient the thing that worries me about this is that someone on the radio said ' the follow ups aren't needed since patients will know if things are going correctly and will contact their doctor if they need to'. So if something goes wrong it's my fault then is it ?
The stupidity of some of these people beggars belief, I wonder why they think people spend years of their life being trained in medical school if anyone can just decide if their treatment is going correctly themselves. They may well be right that in many cases it's just a formality but what about those few where it isn't - how many lives have to be lost before they see sense?

What a bunch of fucking tossers we have running the NHS.

This week, a consultant only costs the NHS ½ of the cost of a GP. They can deal with the post op patients many times quicker – ‘cos they know the likely outcomes and can see a problem so much easier.

So why do they cost so much?

Because ‘funding’ is allocated by cretins in Whitehall, who couldn’t price a tin or beans for Tescos.

What matters is the cost of the whole service. How many Linden Dollars do you think an MRI costs? Who cares, as it comes out of the per capita clinic income. You know, see 20 patients, one costs a whole shed load of money. End of day, break even.

So we take away the £90 per patient (good value if you ask me) follow up for routine patients. Keep £60 and give £30 to GP.

Only patient seen in clinic needs MRI.

Oh shit. We are in mega debt and Patsy is coming to sort it out herself. Otherwise she will fall on her sword.

Ah, I see the light… Let the GPs see the patients for a couple of months….
