What does your own medicine taste like?


Poor little Tory MP (and former nurse) Nadine Dorries has been bleating about the effect of the recent expenses fraud furore is having on the MPs themselves. They don't like being tarred with the same brush as everyone else, and she's worried that some might commit suicide.

Let's recap shall we?


Over the years there have been a number of medical scandals (Bristol, Alder hey, Shipman, Stafford). Often MPs and ministers have been the first to leap onto the bandwagon, condemning everyone involved BEFORE all of the facts are known. There has then usually been a knee jerk reaction 'to stop it ever happening again' which has been invariably ill thought out or positively bad for medicine, research and the public. The Human Tissue Act and the introduction of a 'license to practice' are cases in point.

Doctors have all been tarred with the same brush and some have cracked under the strain, given up jobs or left the country. Some have committed suicide.

Whilst Dr Rant has thus far declined to enter into the fray on the expenses scandal, preferring to sit back and smuggly watch the Schadenfreude unfold, the tempation to stick a pin the walking voodoo doll that is Nadine Dorries was just too great.

However, I would like to offer a word of advice to Ms Dorries - Don't expect much sympathy from your GP.

Benno, when do you want the surgeries open? There are incentives in place at present in my region from the Strategic Health Authority to increase evening surgeries so the working public can access their GP easier. Most people I've spoken to open late one day a week, and don't offer more of a service because it simply isn't used - between 10% and 30% of appointments are usually filled. If you're keen on introducing the laws of business into primary care, you could start with the basic ones, such as supply and demand.

As for the contracts... The Government showed its complete lack of understanding. MPs watched a few TV shows and assumed that consultants spent their lives on the golf course and in private practice, so decided to anchor them to NHS sessions and pay them for it, little realising that most consultants worked 11 hour days and came in at weekends without getting paid for it, simply because they care about their patients.

Meanwhile, they seemed to assume that most GPs were shit at their jobs and a bit stupid, and therefore offering them performance-related bonuses would be cost-effective. It wasn't, because they were already doing 90% of what the QOF wanted because they care about their patients, and thought it wouldn't be difficult to achieve the rest of the 10% with a little organisation.

Of course, perhaps the negotiators for the GPs and the consultants should have done the decent thing and held their hand up at such generous offers, explaining that the DoH was about to piss away a fortune. I'm amazed they didn't - after all, the politicians they were dealing with are clearly very keen on saving the public's money by their principled management of their own finances.

I made that last bit up, obviously.

I love your last paragraph. I love more your "because we care about our patients" Can I wire you up to a polygraph when you say that? Your profession exists to promote itself. I don't blame you at all. The politicians created the monster and have allowed you to morph at leisure. Good luck to you. I don't blame you for extracting as much out of the government as you can. Just don't spin the "we care" line. You have a 1950's based system designed around a 1950s based mentality. And at this present time, you deserve each other. Thanks for the reply-its gone 6 so I expect your OOH colleague will reply now.

Sorry for not replying thus far, but I managed to keep today down to 11 hours, so I can reply now.

Do you work for the DoH? If not, you've missed your calling and a first class seat on the gravy train. You come across as an advanced practitioner in missing-the -original-point fuckwittery.

In the commercial sector, enterprises have the right to tell prospective 'customers' to fuck off. If I could tell people like you to get the fuck off my patient list, I'd work for a lot less.

Never worked for the DH-although I might apply when my surgical fellowship finishes. Can I get a reference?
I am very complimentary about you if you read my post. Nothing wrong with looking after yourselves and then lying about where your priorities lie. Its something to be proud of if you can get away with it. And so far you have.

Cottage industries are all the rage now in England aren't they? Farm shops, Organic Meat, Fishmongers....and then now General Practice.


Your language and that of your friends is a bit accusatory though isn't it? And to think I thought we were part of the educated classes..

Go on.... think of something original to say-rather than flattering me all the time. I am flushed with joy at the thought of it.

We as a society need to get value for money out of our MP's. I rather think that MP's should be given measurable targets to achieve (just like doctors). They should be made to attend monthly performance meetings such as the ones I attend on behalf of my directorate. Those who fail to attend their targets should be publicly named and shamed.

The system would probably work, if only MP's actually had some tangible measurable work to do.
