The wiley DoH are merely replicating tactics routinely used by hospitals whenever a serious complaint of clinical incompetence/negligence is made against them.

The rules a very simple;
Drag the case out for as long as possible, preferably many years.
Lose/mislay key documents.
Spread the blame to avoid personal culpability.

If the above strategy fails make a tantalising cash offer shortly before the case is due to be heard in court - with a proviso that the hospital still refuses to accept any wrong doing.

Issue a dog eared 'sorry' letter complete with empty promises to review procedures [even when staff have been highlighting dangers for years].

If money is involved - especially large sums - it would be extremely niave [sadly] to expect integrity from a kafkaesque and unaccountable government department.

The NHS has innovated in expensive change making.

It has utterly failed to make simple changes that would help doctors and patients.

What a total waste.
