All patients to receive any and all drugs that may possibly help them and the government have put forward £100m for this! ten times that won't even cover the bill, each year!

Lord Darzi is led by the NuLabour government whose agenda is to cause the NHS to fail and blame the doctors and staff for its failing.

Actually the whole of the NHS reforms over the last 18 years (all based on the fatally flawed idea of an "NHS internal market") have been wrong in concept and execution.

We are right to be offside with them, (we retain a functional internal crap detector) and will be delighted when their failure becomes obvious to everyone.

We might then get a chance to knock Nye Bevan off his pedestal, and sort out a modern, probably European model system based on plurality of providers under social insurance.

No other country except Cuba has an NHS...and no other country wants to copy our NHS or our soccer tactics. Perhaps they know something we don't.

And for all Lord Darzi's surgical brilliance I think I have more knowledge of primary care in my little finger than the noble Lord has at all. He's the wrong man to tackle primary care. He'll do as much good for primary care as Beeching did for the railways.

The thing I found interesting was that the Noble Lord seemed to like songs without having really listened to them. Thus the Supertramp "Logical Song" - which he picked, as I recall "because it reminded him of all the time spent studying and swotting" - is actually a song fairly clearly CRITICIZING "intellectualisation" and implying that people should stick with instinct. Can't stand the song, but I did think it oddly instructive that Lord D hears what he wants to hear.

Also amazed that someone who trained outside the UK, and only came to the UK as a consultant SURGEON, should get charged with "fixing" GP care (leaving aside the question of whether it needed fixing by stealth privatisation, which every GP I know is vehemently opposed to). If Darzi had been put in charge of sorting out the kind of internecine warfare and waste of resources in hospitals evident in Gerry Robinson's TV shows a year or two back, that might have made some sense. But GP-land?

Another thing that came over strongly was that Lord D sees himself as a "manager of change". He made several comments about "stuck in their ways" types, so it seems clear how he is casting the GPs... very much in line with Govt thinking.

Yes, the members of the team are (hopefully!) intelligent and articulate doctors, and (hopefully!) our print articles will reflect that.

And, from time to time, Dr Rant will take his neuroleptics and produce a thoughtful, non-anglosaxon post here too.

However, the purpose of Dr Rant is to stand up, rent his clothes, and scream. Dr Rant is a warning to people of what lies ahead on this read. Dr Rant is a wailing lost soul.

You don't negotiate with Dr Rant. You negotiate with moderates in the medical profession because you want to avoid Dr Rant.

So, when you are talking to Darzi and taking a leaf of faith that he might not be a lying stooge out to corporatise the NHS to line big business pockets with the blood of those who have no voice, you can remind Darzi of the alternatives.

When or if we need a medical blog of moderate, sensible opinion then we'll set up a Dr Sensible and Moderate Opinion blog. Until then, let Dr Rant stand as a warning to those who cannot yet see where the road we have embarked on leads.

I like Dr Rant's article in Remedy's new magazine so much. He was softly spoken and, as usual, with stamina and intelligence .. and it suited him better IMO.

And, has it ever occur to anybody that Dr Rant's abilities, other than the anglosaxon stuff, are part of the reason why 'they' chose to came here, rather than to other medical bloggers?

Dr Rant, why are you so silent? This maybe is an opportunity so take it and speak up please 'softly, softly', omit every anglosaxon word that tries to escape for a while. Ability reaching cieling level, I do not know much about the technical stuff Doc Doc is talking about so, you take over please.

Maybe Dr Rant is the 'Jimmy carter' needed for the negotiations to start and until a suitable truce is signed.

Main problem in NHS is secondary care as pointed out above. Also reduced number of hospital beds, 4hr cas target, 13/17 week outpatient targets number of operations/waiting time for ops targets.

Each year more people are seen in cas, outpatients, GP land, more operations are needed, medical advancements and patient expectation are main reason for this, but there is no real year-on-year advancement and development to allow this, just more cutbacks and reduced staffing levels in hospitals.

GP land has expanded to take on more staff since the new contract and fulfilled the objectives set out in that contract.

The changes proposed by Lord Darzi regarding polyclinics are not based on any clinical evidence, while the medical profession has to provide this for any new treatments, research projects etc. Double standards!

Am sure NIKU will point out that number of staff in hospitals has increased but not in the right areas and mostly (bean counting)managers.

Will be intersting to see if Lord Darzi takes on board the petition signed by 1.2 million britons.

doc doc, I think you can safely generalise this statement to include all doctors at the moment, but the current climate of 'us' and 'them' and the constant name calling will never bear fruit.

And, broken down talks lead to nowhere, that's why the situation has decended into anarchy!

The government now needs to show 'proof' that they have good intentions and the doctors need to talk and cooperate.

If Lord Darzi is coming here himself/sending staff here as Niku claims, then he wants to talk. I personally think it would be stupid to waste the opportunity so, everybody needs to tone it down a bit and try some diplomacy, even on the smallest of levels, here on a blog ... you never know.

And Lord Darzi needs to show that he IS one of you somehow by not siding with the government whom he represents all the time. Maybe by demanding that the media hate campaign be stopped or something. But give the man a chance if he is showing readiness to cooperate.

As for ideas, well, you ARE the professionals concerned, come put them down here (Calmly in an adult way) and let's get talking ..
