In the old system a lot less were unemployed as a result of an SHO being an SHO. If you could secure an SpR position you sould have to consdider an alternate career. The rebranding of SHOs and SpRs has created a 2-tier job structure of training and non-training. The latter is obvously far less appealing and thus docs are unwilling to take these through lack of security. The promise of RTT was the biggest problem as it shifted expecations. When you look at it they were essentially offerin CST as long as you didnt fuck it up. The fallout from last year is that nearly all appointed are run-through and hence NTNs have become somewhat saturated for a while.<BR/><BR/>Medical training has been dumbed down by way of assessment tools and competency based training. I feel that the responsibility for 'real' learning is placed on the trainee in the form of self funding for what is mandatory training e.g. ATLS. Instead, jobs were dished out to undeserving, not-very-good doctors who paid 400 pounds to have their MTAS applications filled professionally!
